This manual contains record forms to assist you in maintaining pesticide application records, which are mandated by the Louisiana Department of Agriculture and Forestry (LDAF) and the Environmental Protection Agency Worker Protection Standard. The forms found in this record keeping booklet also include record fields that are required by the labels of a few pesticides due to their unique requirements. 


These forms are designed to assist you in your daily operation. The information that is recorded on these forms becomes the official record. Please be sure to record information legibly and thoroughly. 
Records may be maintained either written or electronically. According to Louisiana pesticide law, your pesticide application information must be recorded within three days of the application and must be maintained for a minimum of two years.
